Gender-affirming care and personalized support
If you’re enrolled in one of the UnitedHealthcare plans—Gold Choice Plus, Platinum Choice Plus, or Silver HDHP Choice Plus with HSA—you have access to LGBTQ+ inclusive care benefits designed to help you feel supported, respected and understood. UnitedHealthcare Advocates provide you with a welcoming, open environment for conversations about your benefits, care needs, claims, authorizations and provider options.
Get support with benefit questions, finding or scheduling appointments, claim information and authorization needs. Your eligible dependents (spouses, domestic partners, and children) can use these benefits, too, if they’re enrolled in the plan.
Gender-affirming care may include hormone therapy, voice modification therapy, certain surgeries and related services when medically appropriate and covered under the plan. Some services, including gender-affirming surgical treatment, require prior authorization and coverage can vary based on your plan documents.

Family formation
A variety of family planning benefits may be covered, depending on the benefits selected by Pandora. Under UnitedHealthcare standard benefits, fertility preservation may be covered if an individual is undergoing gender-affirming surgery that will make them permanently infertile or sterile. This includes egg retrieval or sperm collection, cryopreservation and storage for 12 months. For additional information about family formation benefits, contact a UnitedHealthcare Advisor.

Families grow in many ways. Pandora supports the growth of your family with adoption and surrogacy benefits to assist you—by alleviating some of the cost—with the growth of yours.
Fertility benefits
Through our UnitedHealthcare plans, you have a range of fertility benefits available to you, including:
- Mental health support during your journey
- Fertility treatment coverage—without a medical diagnosis
- Invitro fertilization
- Egg freezing (must be medically necessary)
- Specialized assisted reproductive technologies (must meet certain requirements)
